1. Payment is expected at the time of service, return Check fee is $25.

2. Child must be accompanied by legal guardian at the time of visit. A letter of permission to treat a child signed by legal guardian will be accepted.

3. The first shot record is free. Additional Shot records will be given at a fee of $5 each.

4. Record transfer and or case summary will be charged at $10 or $2 per page which ever is larger, up to $25 dollars. Allow 30 days for record to be processed.

5. We collect your office co-payment at the time of the visit.

6. The office bills your insurance. If the insurance fail to pay the clinic, you may recibe a bill through the mail or just get a phone call. Let the receptionist know if you have more than one insurance valid for your child. It is your responsibility to let us know, wich is your primary insurance!
Dr. Isart and Kids ‘N Teens Clinics staff have “on Call” agreement with-each other. Please note that there may be clinical appointments when you may not see your selected primary care provider at one of the locations because your provider is not in town or if he/she is “over booked”.
